Position Summary

This position provides long term in home support services to pregnant women and families with infants and children under the age of three (3) focusing on information and education related to infant / child development and parenting.

 

Flexible work schedule after an initial training period that includes working in the office and out of the office.

 

Starting Annual Salary: $31,000.00

Essentials Duties and Responsibilities

  • Drives to client homes to conduct initial home visit by completing assessments utilizing the Kempe Family Stress Checklist and completing required initial paperwork if also serving in Family Assessment/Resource Specialist Role
  • With the family, completes written service plan and establishes goals and objectives consistent with the HFA model.
  • Accepts and process referrals as assigned by Program Supervisor
  • Completes all required ongoing paperwork as required by HFA and Funding sources and enters data into the HVOL management system
  • Provides flexible, in-home services to the family including child development and educational curriculum
  • Provides referrals and advocacy to link families with the appropriate community resources
  • Ensures coordination of Healthy Families services with other providers (e.g Public Health Department, medical providers, DHHS, day-care, school system)
  • Plans and participates in monthly parent-groups
  • Performs other special assignments at supervisor’s request

  • Ability to communicate effectively, both orally and in writing
  • Ability to prioritize and self-manage job duties, including timely completion of required paperwork
  • Ability to enter data into Program Management System
  • Ability to speak Spanish

Recommended Employment Qualifications

Education:

A Bachelor’s Degree in Social Work, Child Development or a related field is required with a minimum of one year of related experience in child development and experience in working with infants, young children and their parents or families.

A High school diploma or equivalent is required with a minimum of one year of related experience in child development and experience in working with or providing services for infants, young children and their parents. 

 

Experience:

A minimum of one year of related experience in child development and experience in working with infants, young children and their parents or families with a Bachelor degree.

 

-or-

 

A minimum one years of related experience in child development and experience in working with infants, young children and their parents or families is required with a High school diploma or equivalent.

 

Certificates, Licenses, Registrations:

Infant Mental Health Endorsement preferred.

 

Supervisory Responsibilities:

This position does not have supervisory responsibilities.

 

Special Requirements:
This position requires a valid driver’s license, a good driving record, flexibility in schedule (some evenings). Must submit to Agency approved background checks.

 

Work Environment:
This position is primarily home-based and requires travel to client homes as required by position responsibilities and/or Agency management. This position also requires travel to various community services and may involve the transportation of clients. This position will also be exposed to typical office noise levels and/or exposure to various weather conditions.
